Why
1,5-anhydroglucitol(1,5AG) is a glucose-like substance contained in our body.
When glycemia increase, it is excreated quickly into urine with glucosuria.
If a patient has postprandial hyperglycemic state, she or he begins to have repeated glucosuria, results in decrease in serum 1,5AG level.
Not as like 1,5AG, both HbA1C and glycoalbumin require several hours to form their change, binding with
glucose(glycation). Postprandial hyperglycemia, shown in early stage in patients with diabetes mellitus, is usually a hyperglycemia with short-term duration, often so-called "glucose spike". Thus, HbA1C or glycoalbumin can not catch sufficently this hyperglycemia from the beginning. This is the mechanism which make discrepancy of 1,5AG and HbA1C(or fructosamine) after starting anti-postprandial hyperglycemic drugs, ƒ¿-GI and glinides. !!Caution!! Acarbose is only one exception. This drug often interferes the kinetics of 1,5AG, and lowers its serum level. |
